This online funeral arrangement form allows you to send us all the information we need to arrange a funeral, including information required to register a death in Queensland with the Department of Births, Deaths and Marriages.

Please make sure you complete the form fully and carefully, and take time to check the spelling of names and places. This form can be used for pre-arrangements (including self-pre-arrangements), please be aware the word ‘deceased’ is used when this does not yet apply - no offence is intended.
